Quality and Clinical Manager - Job Opportunity

Starting Salary Range: $31.73 - $33.65 per hour dependant upon qualifications and experience

The Wallace Medical Concern is engaged in significant transition intended to better meet the needs of our community.  The most significant facet of this change involve expanding from an Urgent Care model to a Primary Care Patient Centered Medical Home. To support this work we are implementing electronic health records, and a chronic disease registry.  We are looking for an experienced RN with clinical operations and management skills-preferably in a primary care medical home or safety net clinic setting to join our team!

BACKGROUND

The Wallace Medical Concern was founded on the belief that everyone deserves quality health care.  We have a 28-year record providing patient-focused, innovative services to low-income families in the Portland area who are un- or underinsured and have difficulty accessing care.  (Approximately, 4,800 patients last year)  Our current urgent care capacity is the result of attracting and maintaining volunteer medical professionals, our innovative partnerships and our hiring of a bilingual, bicultural physician for our Mobile Medical Clinic. Our multi-lingual staff works to ensure our care is culturally sensitive.  Our clinics are open at strategic (and mobile) locations.  And we offer evening hours for working people. Our WMC team includes highly qualified professionals, with experience in public health, health administration, and clinical services.

POSITION SUMMARY:

Under the supervision of the Executive Director, serves as a Clinical Operations leader to assist in the achievement of the quality, service, finance, growth and cultural competency goals of the Wallace Medical Concern.  Develops, implements, leads and evaluates systems and processes that drive high quality patient centered clinical care.  Coordinates the quality assurance program for all clinical sites of the Wallace Medical Concern.  Coordinates and ensures compliance with standards set forth by accrediting and funding agencies and identifies, assesses and monitors quality improvement opportunities related to the provision of ambulatory health care.  Assures the Quality Improvement/Utilization Management Program meets the standards set forth by in the Organizations’ Integrated Quality Program.   Develops implements and manages a population-based, culturally appropriate care model for chronic disease management and prevention.

JOB DUTIES AND RESPONSIBILTIES:

General Functions

  1. Responsible for the management of all aspects of patient care delivery including but not limited to patient flow within the clinic, triage, staff competency and training,  case  management and care coordination, education, follow-up and maintenance of required clinical records.
  2. Provides nursing care to patients in a health care facility utilizing the nursing process of assessment, planning, intervention and evaluation.
  3. Assists and coordinates the development, implementation and annual review of the Wallace Medical Concern Quality Improvement Plan. This includes: monthly Quality Improvement Committee meetings, departmental compliance with local, state and federal regulations, development and implementation of policies and procedures related to Federally Qualified Health Centers and the Requirements of the Bureau of Primary Health Care.
  4. Provides recommendations for problem resolution and appropriate follow-up of quality related issues, problems and concerns.  Engagement in problem resolution or program improvement, and proactive risk assessment.
  5. Develops and implements systems and processes to gather and analyze required clinical quality data.  Utilizes the data to prepare and present reports to the Quality Improvement Committee and the Board Quality Management Committee.  Utilizes the information from these reports to inform clinical decision making and operational focus.
  6. Oversees and/or conducts periodic medical and/or electronic health record reviews/audits, and facilitates the completion of periodic peer review audits and other related activities.
  7. Coordinates and oversees the CLIA laboratory services.  Ensures the clinic is compliant with CLIA regulations. Develops policies and procedures and necessary corrective action plans, and coordinates annual lab proficiency testing for all appropriate clinic staff and volunteers.

QUALIFICATIONS, SKILLS AND ABILITIES

  1. Knowledge of current local, state and federal laws and regulations and accreditation standards related to the delivery of ambulatory health care, safety and risk management.
  2. Knowledge of Joint Commission on Accreditation of Health Care Organizations or other accrediting agency issues and requirements.
  3. Ability and experience to manage and supervise subordinates.
  4. Experience in case management especially chronic disease and preventive services.
  5. Ability to communicate effectively and persuasively, both orally and in writing.
  6. Ability to organize, develop, implement, monitor and evaluate professional work plan goals and performance objectives.
  7. Ability to develop, implement and manage individual projects and those of the QI Program.
  8. Ability to perform effectively in a culturally diverse work environment.
  9. Excellent interpersonal skills and ability to work effectively with others in the workplace.     
  10. Experience with low-income populations, Medicaid, Medicare in a community health clinic setting is desirable.
  11. Proficient conversational Spanish is highly desired.
  12. Must adhere to agency’s non-discrimination policies.

  18. Experience with Electronic Medical Records .

WORKING CONDITIONS

Duties are performed primarily in an office and clinic environment.  Some evening or weekend hours may be required. 

PHYSICAL REQUIREMENTS

Ability to work in a fast-paced environment. Requires ability to sit, stand, lift, carry, walk, read find print, and communicate in person, in writing, by telephone and electronically.
