Description: Housing Resource Broker supports brokerage participants to transition into community housing through the use of person-centered planning and self-directed support funds. He/she helps participants assess their current housing situation, strengths and needs and create a plan for successful transition to new housing.  Coordinates entrance into brokerage services with customer, customer's family/advocate(s), and county mental health provider.   Knowledge of:  -Local housing market and housing programs.  -Community resources including, but not limited to Social Security Disability and Social Security Income, Medicaid, OHP, Medicare, Department of Vocational Rehabilitation, housing, transportation and low income resources and other generic community and disability-related resources. - The brokerage service model. - Mental illness and the effects of mental illness. - The core principles of self-determination and the Consumer/Survivor Movement and its history   Duties include: Maintains regular contact with each customer assigned. (At least monthly) - Treats all customers with dignity and respect. - Provides technical assistance to customers to enable them to make informed decisions about support needs and to direct support providers. - Identifies and ensures access to culturally/linguistically appropriate support services to meet customer's specific needs. - Provides assistance for customers to effectively put plans into practice - Guides and facilitates the development of customer's self-advocacy skills as needed and desired by the customer. - Assists in setting realistic budgets and ensuring that purchasing procedures are followed in an accurate and timely manner. - Provides customers with technical assistance in purchasing and securing needed services, including how to utilize and manage Personal Care Attendants. - Maintains current knowledge of local housing market, housing programs, and other housing resources.

Qualifications: - Identify as a psychiatric consumer/survivor/ex-patient or person with mental health issues. - Experience working with at-risk transition age youth (TAY) and adults with mental health diagnoses. - Bachelor's degree in a human services field and at least one year experience in the area of mental health; or five years of equivalent training and work/personal experience related to mental health. - Be at least twenty-one years of age. - Understand and be committed to whole life planning as a means of recovery from mental health issues   Salary range: $16.50- $18.00/hr DOE 32 hours per week.
