Required Skills and Abilities:Skills and experience providing care management for Seniors and adults with disabilities. Valid driver's license and reliable source of transportation. Excellent communication skills. Experience doing assessments and working with multidisciplinary team to develop a service plan. Familiarity with in-home cae rules. Ability to work with limited supervision. Ability to problem solve and come up with creative solutions.
Qualifications:Bachelor's Degree. Master's Degree preferred but not required. 3-5 years care coordinaton, care management, case management experience. Valid driver's license
