Bilingual English/Russian HealthCorps Position- Patient Navigator

Who are we: We are over 18 AmeriCorps volunteers serving at Sea Mar Community Health Centers throughout Western Washington as well as International Community Health Services in Seattle. Sea Mar Community Health Centers is a community-based organization committed to providing quality, comprehensive health and human services to diverse communities, specializing in service to Latinos.

Position Description: Members will work with providers and other clinic staff to assist patients to access appropriate care, minimize no-shows, assure pre and post visit needs are met, facilitate brief education encounters related to chronic conditions, medically complicated cases and for patients with multiple barriers to care within the medical clinic setting. Provide translation/interpretation and coordinate charity drug program assistance. Patient Navigator position duties may vary as each position is customized to meet the specific needs of the Clark County Clinics.
