Healthy Kids Outreach and Enrollment Worker

The Washington County Commission on Children and Families is seeking one half time (up to 20 hours per week) Outreach and Enrollment Worker for the Healthy Kids Project in the Tigard-Tualatin and Sherwood School Districts.

The Healthy Kids Project will implement a strategy for outreach to and enrollment of hard-to-reach children and families in the Oregon Health Plan based on a school district-based home visitation and in-home assistance model. This strategy will provide families with uninsured children who may be eligible for the Oregon Health Plan or other Healthy Kids subsidized health insurance: 1) community-based, targeted outreach, 2) one-on one education about Healthy Kids eligibility and benefits and 3) in-home enrollment and retention assistance. Staff will be housed at the school district Family Resource Center.

Outreach Worker responsibilities include: • Attending trainings • Maintaining internal referral networks in the districts and community • Accepting referrals from school personnel and community agencies • Attending school events that offer opportunities to provide outreach to families • Providing community-based outreach and education at churches, businesses, summer, food sites, low income housing sites, community organizations and various community events • Scheduling appointments, conducting home visits and providing in-home enrollment assistance to help families complete paper-or on-line OHP applications • Completing online application tracking forms and maintaining client records • Preparing weekly and monthly activity reports for submission to the Commission • Participating in forums for coordinated outreach network and in local enrollment campaign.

The person hired will work up to 40 hours every two weeks. Salary is $16.75 per hour, increasing to $17.17 per hour effective July 1st. This position provides no health insurance, holiday, sick time or vacation benefits, but it could continue for up to five years.
