position summary:

The Social Worker is responsible for social work services delivered by PRO. These include education in the field of psychosocial issues and available community resources as well as advocacy for the Parkinson’s patient and their families in the larger community.

The social worker should use his or her professional skills to assess, intervene, and evaluate psychosocial needs. The social worker shall use the trusting and empowering relationship with the client to expedite the use of services along the continuum of care. The goal is to restore or maintain the client’s independent function to the fullest extent possible and maximize quality of life while dealing with Parkinson’s disease.

Social worker provides short term case management and counseling services, assists with daily I&R, plans and implements community education, and cultivates professional networking in the community. The position is based in Lake Oswego, although regional travel (2-3 days/month) should be expected.

Salary / Pay Rate:DOE
Required Skills and Abilities:Ability to work independently with a drive to support the success of others. Understanding of individual, family and group functioning. Excellent verbal and written communication skills. Comfort working with clients with chronic progressive illness.
Qualifications:MSW with experience in health care, LCSW strongly preferred.
