position summary:

Full time, non-exempt status. The Aging Services Case Manager is responsible for providing Case Management services and Information and Assistance to older adults in accordance with Multnomah County Aging Services Division Case Management Standards. Minimum qualification include Bachelor’s degree in Social Services, Nursing, Health Gerontology or related field; MSW preferred. 1 to 2 years experience in case management field, preferably in working with elderly. Ability to communicate with clients, families, service providers, community members and co-workers, both orally and in writing. Knowledge of human behavior and the aging process. Knowledge of social and health intervention techniques. Culturally responsive. Initiative and ability to work as a team player. Must have valid driver’s license, 5 years driving history, personal vehicle, provide proof of personal auto insurance and able to drive agency vehicles. Must pass a criminal background check.

Salary / Pay Rate:Starting pay range is 11.96 to 14.35/hr.
