JOB SUMMARY:  The Resident Services Coordinator I is responsible for planning, coordinating and providing an array of social services for residents with the goal of improving the stability, quality of community life and wellness of families and persons who are elderly and/or disabled in four low-income housing developments.  The Resident Services Coordinator I will rotate between four Housing Authority of Portland (HAP) sites (Dahlke Manor, Holgate House, Northwest Tower, and Unthank Plaza). 


JOB RESPONSIBILITIES:  Assist with the development of a program service delivery model. Link residents with appropriate health, psychological and social service providers. Develop and implement a system to provide follow-up and needs assessments for residents seeking services. Assess the needs of the building community to organize and provide needed services. Perform information and referral service, as well as direct client advocacy. Establish and maintain linkages between residents, family members, housing staff, medical staff and service agencies until presenting needs or issues are satisfactorily addressed. Coordinate with service organizations in the community and assist residents to build informal support networks with other residents, family and friends. Respond to and intervene in resident problems and crises as identified by housing staff and residents. Where appropriate, assist residents and housing staff with site activities. Participate with other service providers, staff and volunteers in coordinating site services. Maintain case files and resident information as required. Perform related duties and responsibilities as required. Completes accurate, legible and timely documentation of services delivered in accordance with Portland Impact’s management information system. Ensures compliance with agency’s and funder’s documentation and evaluation requirements that support quality client services.   




·        Associate degree or equivalent in social work, psychology, mental health or related field
