CCC Community Engagement Program/Housing Rapid Response looking for a Case Manager to work as part of a team with frequent criminal offenders.  Position involves collaboration with various community agencies to assist clients in obtaining housing, and eviction prevention, benefits, employment, mental health, medical and substance abuse treatment.  Applicant must have minimum of a high school diploma, advanced education preferred.  Must have knowledge in and experience working with persons experiencing substance abuse and mental health disorders.
